He dresses as a girl to avoid being found out obviously. However, in the game there is a point where he actually becomes female and has breasts, which I'm guessing is what this image is based on.

spoilerI haven't played it so I don't know ;p I'd guess something randomly magical or something, that'd be the usual route for something like this to happen (there's already a ghost girl in the game/anime so magic really isn't that much of a stretch).
